What Waveapps' application asks

Across the 4 application forms Otto has filled at Waveapps: 20 distinct questions, 12 of them usually required. A single application works out at about 4 minutes once you have your details to hand. It runs on Lever.

7 are Waveapps’s own, not boxes every form has:

• what is your desired salary? (please write down a number)
• if you require any type of accommodation during your interview process and/or while performing the role…
• if you were referred by a wave employee, please provide the name of the person who referred you here.
• acknowledged and agreed.
• are you legally eligible to work in canada with no restrictions (e.g. pr, open work permit, etc.)?
• are you located in canada?
• yes

Read off the forms themselves, last checked 2026-09-18, because Otto fills them in and has to read them first. Roles differ, so the estimate is per application rather than for the list above: each question counts for its cost times how often this employer actually requires it. A written answer is 2½ minutes, a dropdown 12 seconds, a yes/no 5 seconds, attaching a file 45 seconds. The list leaves out name-and-email boxes and anything Lever puts on every employer’s form.
